Small spoiler regarding my realm state:
This is my T2 farm realm: Falstead defeated in The Black Trench but, before talking to the Madrigal in The Nameless Pass.
This chest usually spawns with some crates behind it, and I can never open it. I have run this area several times farming for T2 gear.

Realm State: defeated riven twins, yet to do crucible.
Came here to report that same chest.
the chest underneath the first bridge when entering nameless pass also often buggs out and is not “interactable”
they often can be opened if you are persistent and stubborn enough to try it long enough 
I think the proximity detection or whatever it is called is a little too small, or offset in an unfortuante way
