At some time point in 4-21th, my shield in my first off-hand slot suddently became unavailable. The shield was surely equiped in the slot, the shield icon was in my first off-hand slot too, but in game it did not work and no icon of off-hand shield showing in left-down UI as well as my parry was done like without shield.
I can only put my weapon and shield in the 2nd main-hand and off-hand slots and everything works.
So i was playing the game in a crucible and i noticed that out of nowhere my bow stopped throwing “lightning” arrows.
I checked if i accidentally unequipped the quiver but it was still there, i tried to unequip and equip again nothing happened.
I changed the quiver to off-hand slot 2 and my bow started working again with lightning arrows. Is this a bug? Did it happen to someone else?

I suspect you have cycled through your off-hand slots to the second one. Can you please check which keybind/button is set for “offhand item”, press that and equip your quiver in slot nr1 again.

Oh my god… this is so embarrassing. You are right, i pressed the “switch off-hand” button on my controller by accident without even realizing this was a thing.
The problem is fixed, it is not a bug. Thank you very much
2 years later and I also just got this bug. Tried everything to fix it that’s within my power. Seems hard baked into my character.
I was just playing the game normally, doing the crucible with my brother, when my shield just vanished right before my eyes.
Now, I don’t mean the item itself. My shield is still equipped, but the off-hand slot has been rendered useless. The character sheet doesn’t even pick up the weight of the equipped shield or torch. The off-hand slot is effectively dead.
I have done everything in my own power to fix the issue, unequipped the shield, logged out, restart with and without weapons on. For lack of better words, it’s like the bug is hard baked into my character.
In this link, I have uploaded a video of the issue and my character save files, in case you want to take a look yourself, if that’s possible, and all other requested data.
Make sure you have not cycled through to your second or third off-hand slot please!

I made a fresh ticket on here some where with a video show casing the bug. If you can find it shows it’s definitely a bug of some kind.
Please navigate to the keybind/button “off-hand item”, use/push that and find your shield again.
