The world outside of Sacrement should have randoms town and routes where you meet npc’s with different side quests. I would say something along the lines of Fable 1 or Witcher 3. I would suggest implementing a character level scaling; for example you are lvl 1 and your coop player joins and is much higher. We should both be doing and receiving the same amount of damage along with shared loot possibly. Borderlands 3 and 4 have this in their game. I shouldn’t have to create a whole new build just to help out a player of a different level.
