In short, I found that while killing enemies I accidentally killed them with damage reflect on one of my items (Reflect 3% Damage Taken) and it counted towards the challenge, which specifies killing Scoundrels with Spears.
Find appended three different instances (one done fully without fire for testing if its a factor.)
