Poise/stagger on hit?

What triggers or adds to the effect? The physical hit of the weapon or the damage dealt?
If it’s the hit, is the base amount distributed between every hit of an attack animation and therefore the effect will be lower when missing some hits, compared to one hit weapons?

When you see the info of a weapon there is a number next to the damage with a half broken-shield icon (upper right). That’s the “poise damage” and I think it’s a base number by each hit because you could do more with a charge attack.

It is triggered by each physical hit and not the damage dealt. Usually fast weapons have lower poise damage (because you land more hits per time and hits are harder to dodge) than heavier-bigger weapons. I.e two handed axes and clubs deal three times more poise damage than a staff or a scythe (and are slower also). Bear in mind that you could increase attack speed but that wont change poise damage.

So there is a relation between weight and poise damage, and not with damage (adding a oil blade increases your damage while poise damage remains the same).

Poise damage also depends on armor (heavier armor has more poise resistance), you can improve your poise damage with enchantments and some runes deal more poise damage than others. I.e there is a rune from a one hand sword (don’t remember the exact name) that it’s focused on staggering.

Shields deflect poise damage.

I supose that the base number of poise damage may depend on other things as attributes and weapon upgrades and so on, but I am not sure about that.
