In the sewer-type Crucible room with the cages, I knocked an enemy off the side and it seemingly didn’t die from falling, or something went wrong with the spawning because after killing all the enemies, the door to the elevator didn’t open. So I became soft-locked in the room with the only way to escape being to kill myself jumping off the edge and waste a fallen ember.
Luckily it was the first room in the run so not too bad but if it had been in a later room it would’ve been seriously annoying…
