This would be anything that you have seen only once or twice, armor or weapon.
I’ve only seen one of this:
Had a bunch drop before the Breach but for some reason, never after the Breach except once.
now that I think about it, I should be missing the helmet too… they probably put it as a unique drop for the plague doctor.
for the armor, I think the rarest pieces are those of Darak’s armor since I think they can only drop from him and only in story mode when you kill him (so you have to create more realms, the plague version never gave them to me).
as for the weapons, some are definitely much rarer than others

But for real, the Caretakers staff, probably.
Someone should make a list of rare drops so we can see if we have them or not, lol.
