During the Plagued Knight’s “3 sword nova” attacks, he teleported inside a wall and then died.
His body is inside the wall and the loot is unreachable.
He was on fire at that time, so I can’t tell whether he teleported into the wall because he died during the attack animation, or if he died because he teleported in there.
